Included care
- Initial comprehensive and periodic examinations.
- Up to two routine cleanings for healthy gums. Scaling and root planing (deep cleaning) is not included.
- Initial full-mouth X-rays; panoramic image at first recall.
- Up to two fluoride treatments for ages 12 and younger.
Additional savings
10% discount on additional cleanings, emergency visits, aesthetic and general dentistry procedures, and additional diagnostic exams/X-rays. Orthodontics excluded.
Membership terms
- No waiting period, deductible, or annual benefit cap.
- Complete included services and discounted treatment within the applicable plan period; confirm dates with the office.
- Added family members share the original expiration date.
- Cannot combine with insurance or other offers.
- Nontransferable and nonrefundable.
- Annual automatic renewal; ask about renewal and cancellation procedures before enrollment.
